After your first appointment has been scheduled, please print and fill out the forms below to bring to your first appointment. This will save time that we can use to talk about your reason(s) for entering into therapy instead of using it to fill out paperwork.

If using teletherapy sessions, please send the completed forms by attaching them to my secure email at [email protected] ​ (One suggestion is to save the completed forms in a folder on your desktop to send together.)
